Intermittent sudden dyspneic episodes.

چکیده
Volume 87, Number 3 www.aafp.org/afp American Family Physician 217 A 62-year-old nursing home patient presented with dyspnea. He had recently been discharged from the intensive care unit after treatment for urosepsis with multiple organ failure. While in the hospital, he required vasopressors and mechanical ventilation for more than three weeks. After discharge to a skilled nursing facility, the patient had several episodes of acute respiratory failure. The episodes were sudden and self-limited, resolving rapidly with minimal intervention, such as nebulizers, supplemental oxygen, or head repositioning, and were associated with clear lungs on auscultation. The episodes occurred when the patient was recumbent with his head slightly elevated, or when he was sitting and reached forward. Imaging was performed (Figures 1 and 2).

Performance of electrophysiologic study in an asymptomatic patient with type 2 intermittent Brugada syndrome: To do or not to do?
Background: Brugada syndrome (BrS) is an inherited channelopathy, which is associated with sudden cardiac death due to rapid polymorphic VT or VF. There is no definite consensus regarding the management of asymptomatic patients. Some experts advocate close follow-up; others propose the programmed stimulation for risk stratification. We aimed to evaluate the benefit of complete atrial and ventri...

A 5 year old girl presenting with episodes of sudden loss of consciousness was found to have intermittent ventricular tachycardia and, on one occasion, self limiting fibrillation. Corrected QT interval was normal. After several therapeutic measures clinical and electrocardiographic improvement was achieved by administration of sotalol.
متن کاملThe Impact of Verapamil on Catecholamine Polymorphic Ventricular Tachycardia
Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a disease of mutation of family genes associated with exercise-induced syncope or sudden death. We report a case of a 27-year-old female with CPVT who had experienced several episodes of syncope and had been prescribed beta-blocker regularly. Sudden onset of ventricular arrhythmia occurred and the patient was resuscitated but remai...
متن کاملReversible blunting of arousal from sleep in response to intermittent hypoxia in the developing rat.
Arousal is an important survival mechanism when infants are confronted with hypoxia during sleep. Many sudden infant death syndrome (SIDS) infants are exposed to repeated episodes of hypoxia before death and have impaired arousal mechanisms. We hypothesized that repeated exposures to hypoxia would cause a progressive blunting of arousal, and that a reversal of this process would occur if the hy...
